EPIC+ Top 10 Violations

Members of the EPIC+ Workgroup have conducted annual workshops based on the top 10 most common violations found at San Diego County Biotech and R&D facilities from 2004-2010

 

 2010 Top 10 Violations
  1. Hazardous materials business plan incomplete or not amended.
  2. Hazardous waste container missing label or not properly labeled.
  3. No annual carcinogen/reproductive toxin list submitted to HMD.
  4. Medical waste management plan not submitted to HMD
  5. No generator info on red bags/sharps containers.
  6. Hazardous waste containers not kept closed.
  7. Records of hazardous waste disposal, including manifests, not maintained for 3 years.
  8. Medical solid waste not secured to deny access by unauthorized persons.
  9. Hazardous waste stored >90, >180, or >270 days.
  10. Failed to comply with satellite regulations.
      
 2009 Top 10 Violations
  1. Hazardous materials business plan incomplete or not amended.
  2. Hazardous waste container missing label or not properly labeled.
  3. No annual carcinogen/reproductive toxin list submitted to HMD.
  4. Medical waste management plan not submitted to HMD
  5. No generator info on red bags/sharps containers.
  6. Hazardous waste containers not kept closed.
  7. Hazardous waste container in poor condition (added 2009)
  8. TSFD-signed manifests not on site.
  9. Hazardous waste stored >90, >180, or >270 days.
  10. Medical waste red bags not containerized.
     
 2008 Top 10 Violations
  1. Hazardous waste container missing label or not properly labeled.
  2. Hazardous waste containers not kept closed.
  3. TSDF-signed manifest not on site.
  4. Hazardous material site map not sufficient.
  5. Hazardous material business plan incomplete or not amended (added for 2008).
  6. Hazardous material business plan inventory incomplete or not amended.
  7. Hazardous material business plan not available for review (added for 2008).
  8. No annual carcinogen/reproductive toxin list submitted to HMD.
  9. No generator info on red bags/sharps containers.
  10. Medical waste management plan not submitted to HMD (added for 2008).
     
 2007 Top 10 Violations
  1. Hazardous waste container missing label or not properly labeled.
  2. Hazardous waste stored >90, >180, or >270 days.
  3. TSDF-signed manifest not on site (added for 2007).
  4. Hazardous waste containers not kept closed.
