The Health Promotion unit educates and motivates older adults about taking responsibility for their health through a lifestyle of good nutrition and fitness. Two major focus areas of the Health Promotion unit include fall prevention and diabetes. A major goal is to increase opportunities for older adults to participate in physical activities.
Representatives from a network of senior, health and other community organizations meet monthly to exchange information and cooperate in collaborative programs.

Feeling Fit Club, an exercise program for older adults:
The Feeling Fit Club is a functional fitness exercise program offered via three different delivery methods throughout San Diego County: 1) on-site classes with trained instructors at various community sites, 2) a television program that is shown twice daily on three stations, and 3) a video/DVD program for home use. The classes include 4 core components: strength (upper body, lower body, and core), balance, aerobic endurance, and flexibility. All moves and exercises are designed to be adapted to various physical abilities and can be performed from a seated or standing position. Program evaluation has shown improvement in participants’ strength, flexibility, balance and ability to perform activities of daily living. Please see the current schedule.
The exercises in the home based program are focused on more frail seniors who are home-bound. There are five unique episodes which build in difficulty from Monday to Friday, and the episodes are repeated each week. The television programs are shown twice per day, Monday through Friday at 8:00 a.m. and 1:00 p.m. Cox: 19 or 24; Time Warner: Channel 124; Adelphia: Channel 66. Please note that the program is shown on the “Local Government Access” station and may be listed in the TV guide as Feeling Fit – it may show up as “Local government Programming” and it is sometime pre-empted by other programs. We have no control over this.
For seniors for whom this schedule does not fit their daily routine, of if they are frustrated with the show getting pre-empted, the VHS video or DVD can be sent to them along with a stretchband for home exercise. The DVD/Video is exactly the same as the TV show (all 5 episodes).

Chronic Disease Self-Management Program (aka, “Healthier Living: Managing Chronic Conditions”)
Designed at Stanford University and for people with conditions such as arthritis, depression, heart disease, diabetes, COPD, or any chronic illness, this program has been proven to achieve positive health outcomes and reduced health care expenditures. The program consists of a workshop that meets 2 ½ hours per week for six weeks, led by two trained peer educators, who also have a chronic condition. Topics include diet, exercise, medication management, cognitive symptom management, problem solving, relaxation, communication with healthcare providers, and dealing with difficult emotions. The Healthier Living workshops are available at sites throughout the County.

Diabetes Self-Management Program (aka, “Healthier Living with Diabetes”)
Designed at Stanford University and for people with Type 2 Diabetes, this program has been proven to achieve positive health outcomes and reduced health care expenditures. The program consists of a workshop that meets 2 ½ hours per week for six weeks, led by two trained peer educators, who also have diabetes. Topics include diet, exercise, medication management, blood glucose monitoring and management, foot care, complications of diabetes, problem solving, stress reduction, and communication with healthcare providers. The Healthier Living with Diabetes workshops are available at sites throughout the County.

The Health Promotion unit has developed an expanded version of the Healthier Living with Diabetes program which includes the Stanford workshop PLUS two individual consultations with a Registered Dietician on diabetes educational needs and nutrition before the workshop as well as post-workshop follow-up by program staff. We expect this program to become a recognized Medicare diabetes self-management program in 2013.
Fall Prevention Programs:
Scientific evidence indicates that comprehensive fall prevention for older adults include management of medical risk factors, environmental safety, safe behaviors, and exercise that focuses on strength, flexibility and balance (such as Tai Chi). Our website, www.SanDiegoFallPrevention.org offers a Toolkit and Resource Guide, including contact information for local organizations that address different facets of fall prevention. The website also has a variety of educational videos, including English and Spanish videos for seniors, and short video for clinicians.
As funding permits, we offer three evidence-based fall prevention programs: Matter of Balance, Stepping On, and Tai Chi: Moving for Better Balance. However, these programs are not enrolling at this time. Other classes may be found on the website resource guide. (www.SanDiegoFallPrevention.org)

Fall Prevention Task Force: Mission: Reduce falls and their devastating consequences in San Diego County. Meets on the 2nd Tuesday of each month from 12:30 – 2:00 at the Burn Institute: 8825 Aero Drive, San Diego 92123. Task Force members work on special projects, do presentations in the community, and work with AIS on our fall prevention programs. North County chapter meets in Carlsbad on the 3rd Tuesday of each month from 1:00 – 2:30.
