Breakdown Notifications
Breakdown notifications can be submitted online or via email. To learn how to

Alternatively, you may submit the breakdown notification by
calling the Compliance Division at (858) 586-2650, either
during office hours or after hours.
When reporting a
breakdown, please include all of the following information.
- Time of the call and the time of the start of the breakdown
- Estimated duration of the breakdown, if known
- Specific location
- Equipment involved, and
- To the extent known, the cause(s) of the breakdown
Hazardous Materials Business Plan
For Hazardous Materials Business Plan (HMBP) Plan Check, submit your
information online using the Online Hazardous Materials Questionnaire
through Accela

You will be asked to create a username and password and will be able to submit information online. You will have the ability to pay the fee online and receive a digitally stamped copy of the Hazardous Materials Questionnaire. HMD can also refer the information to the APCD for their review and stamp.

Once received, fee payment can be arranged online.

we’ve received your fee payment, you will receive your Hazardous
