County of San Diego Waiver Checklist
Documents
- Checklist Timeline
- County of San Diego Checklist Attestation Form
- CDPH documents:
Schools serving grades TK-6 that are not already open may resume in-person instruction if they:
- Complete and post a COVID-19 Safety Plan (CSP) to their
website homepage
- The CSP includes the School Guidance Checklist and the Cal/OSHA COVID-19 Prevention Program (CPP)
- District sends their CSP to the state (k12csp@cdph@.ca.gov) and completes the County of San Diego Checklist Attestation Form on the same day
- Receive subsequent approval from the County and the State.
- Please note: this process will be in effect for as long as the County of San Diego is in the Purple Tier and with a case rate at or below 25 cases per 100,00 (adjusted rate). If the county moves to the Red Tier, and stays in the Red Tier for 5 consecutive days, schools may reopen without a checklist attestation approval.
Feedback regarding your application will be provided to you within 7 business days of the submitted date. Incomplete information will require a follow-up or re-submission of your application and the timeline will automatically restart. When selecting a reopening date it is recommended that schools and district allow time for possible resubmission (3 weeks after submission date is encouraged). Forms cannot be saved. Please carefully read and complete each question and criteria. Please have all necessary information and documentation ready for submission. Once the checklist and materials are approved, your information will be posted on the County of San Diego website, http://Coronavirus-sd.com/K12Schools.
Please refer to the Schools.COVID19.ca.gov website and email us at COVID-Education@sdcounty.ca.gov If you have additional questions.
