The County of San Diego and its partners are offering vaccinations to individuals in eligible priority groups.

 

Appointments and proof of eligibility are required at most sites.  Space is limited and appointments are updated regularly. Please continue to check this page for new listings.

 

Sharp - South Bay Super Station

Appointment Instructions for Sharp - South Bay Super Station

PLEASE NOTE: It is recommended that you use a browser other than Internet Explorer to complete your appointment registration.

  1. Select a date and time for your COVID-19 vaccine dose
  2. This is NOT a drive-through location. This is a walk-up location at 565 Broadway, Chula Vista, 91910. By appointment ONLY, no walk-ins. This location is NOT at Sharp Memorial Hospital.
    • Clinic Address: 565 Broadway, Chula Vista, 91910
    • Limited free parking is available at clinic site.
  3. Space is limited at the Clinic. Please do not arrive sooner than 15 minutes before your appointment or remain at the clinic beyond your visit. Wear a t-shirt to allow for easy access to the vaccination site. Bring a jacket as you may be waiting for your turn outdoors.
  4. At the time of your appointment, you must present photo ID/documentation to show you meet the eligibility requirements.

Documentation to show proof of employment in eligible sector should include:

    • Employee or volunteer ID card; OR
    • A paycheck stub or timesheet issued within the last 90 days; OR
    • A letter on employer/company letterhead stating that the named individual is a permanent, probationary, temporary employee or volunteer
  1. You will be required to wear a mask and adhere to social distancing guidelines for the duration of your visit.
  2. After your vaccine has been administered, you will be asked to remain at the location for a 15-minute observation period.
  3. Before leaving, you will be provided a vaccination card with the date of your next dose.

Appointment Instructions for Operation Collaboration Vaccination Events

PLEASE NOTE: It is recommended that you use a browser other than Internet Explorer to complete your appointment registration.
  1. Select a date and time for your first COVID-19 vaccine dose.
  2. Current Operation Collaboration locations are walk-up sites, by appointment.  
    • Clinic Address: Multiple locations
    • Limited Parking available at each clinic site.
  3. For everyone’s safety and to adhere to social distancing guidelines, please do not arrive sooner than 15 minutes before your appointment or remain at the site beyond your visit.
  4. At the time of your appointment, you must present photo ID/documentation to show you meet the eligibility requirements.
