UnitedHealthcare/UMR Select Plus - HDHP/HSA
The High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) is a unique medical plan that puts you in control of your health care spending.
Doctor/Health Care Providers: You can choose any doctor you want, and you can go to any hospital or pharmacy. However, you'll pay less when you use a provider or facility that participates in the UHC/UMR PPO network. This is the same network used with the UHC/UMR Select Plus PPO Plan.
Annual Deductible: You generally pay an annual deductible before the plan begins to pay for a portion of covered medical services.
Coinsurance: When you receive medical services, you pay a percentage of the cost of the service, and the plan pays the remaining percentage. You will need to pay the annual deductible before the coinsurance applies.
Annual Out-of-Pocket Maximum: The UnitedHealthcare HDHP includes an out-of-pocket maximum. This is the maximum amount you must pay out of your own pocket for the annual deductible and coinsurance combined. Once you reach the out-of-pocket maximum, the plan pays 100% of in-network charges for the remainder of the plan year.
Prescription Coverage: Prescription benefits are managed by OptumRx. You will receive one ID card from UHC/UMR for both medical and prescription drug benefits.

The HDHP with an HSA allows you to save money toward out-of-pocket expenses now and in the future. You can use HSA funds for any IRS qualified medical, dental, and vision expenses.
You can save up to the following limits in 2022:
• Employee only coverage: $3,650.00
• Employee + 1 or more coverage: $7,300.00
• Catch-up Contributions - If you are age 55 or older, you can make an additional "Catch-up" contribution of $1,000.
The minimum contribution amount is $5.00 per pay period and $120.00 annually.
The County may also contribute to your HSA if you have excess Flex Credits. If you enroll in a High Deductible Health Plan, any excess Flex Credits are automatically placed in your HSA. The limits above include both contributions from you and the County.
The HSA is not established or administered by the County — you must set up your HSA. If you want to contribute to your HSA through payroll deductions, you must set up your HSA through Optum Bank.
