We have an online referral process for professionals and mandated reporters to make referrals using the links below:

Adult Protective Services:

Using the online referral system is very easy, and you don’t need to fill out any other forms anymore.  All you need to do is use the system online.

In-Home Supportive Services (IHSS) and Care Management Programs Referrals

To make a referral to any of the following organizations follow the instructions below:

·      In-Home Supportive Services (IHSS)

·      Multipurpose Senior Services Program (MSSP)

·      Senior, Options, Advocacy and Referrals (SOAR)

·      Linkages

1. Register at the following link and create a username and password:  https://sd.boundsportal.net/Portal/SDPublicSignUp.aspx
