• VSD-001 (Filled Out)
• Copy of DD-214 (Member 2 or 4) with no redactions
• Copy of current Driver's License
• Copy of evidence (VA Decision Narrative Letters, Service-Connected code sheet, etc.) showing that one of the criteria listed on VSD-001, Section 4 have been met:
  • The veteran has a disability which has been rated at 100% due to a diagnosed disease or disorder which substantially impairs or interferes with mobility
  • So severely disabled as to be unable to move without the aid of an assisted device
  • Lost, or lost use of one or more limbs
  • Suffered permanent blindness, as defined in Section 19153 of the Welfare and Institutions Code
