OMVA Appointment Checklist
The following checklist will help you gather the information you will need to file a Compensation, Pension, Dependency and Indemnity Compensation, and/or Survivors Pension claim.
Once materials have been gathered, please call 1-866-726-8831 or email us at sdcvso.hhsa@sdcounty.ca.gov.
Compensation Claims
- Original or Certified copy of DD Form 214 member 2 or 4
- Copies of all, if any, current relevant private medical records
- Direct Deposit Information including account number and bank routing number
Pension Claims and Survivors Pension Claims
- Original or Certified copy of DD Form 214 member 2 or 4
- A list of all current income and net-worth information, to include but not limited to social security benefits, retirement income, and dividends
- A listing of all current medical expenses
- Direct Deposit Information including account number and bank routing number
- Copy of Veteran's Death Certificate (for Survivors Pension)
- Copy of Marriage Certificate to show dependency (for Survivors Pension)
Dependency and Indemnity Compensation (DIC) Claims
- Original or Certified copy of DD Form 214 member 2 or 4
- Copy of Veteran's Death Certificate
- Copy of Marriage Certificate to show dependency
- Direct Deposit Information including account number and bank routing number
- Previous marriage(s) information (if applicable): to include ex-spouse(s) names and date/location each marriage began and ended
