Employer Forms

The most commonly used forms are provided below. By completing and selecting Submit, these forms will be sent electronically to San Diego DCSS.

Employee Status Report - use to confirm receipt of Income Withholding Order (IWO).

Notification of Termination - use to tell DCSS an employee has been terminated.

Wage and Insurance Verification - use to provide DCSS with employee wage and insurance information.
