The Office of Vital Records and Statistics (OVRS) is excited to announce its new location.

The County of San Diego is a part of the State Medical Marijuana Identification Card Program. This program was created to make a state-authorized medical marijuana identification card (MMIC), along with a registry database to verify qualified patients and their primary caregivers. By law, this card can be used to verify that a patient and/or caregiver can have, grow, transport and/or use medical marijuana in California. Signing up for this program is voluntary for patients and primary caregivers.

Who Can Apply

Talk to your medical provider to see if you qualify for an MMIC. To qualify, you must have a serious medical condition. This includes:

  • AIDS; 
  • Anorexia; 
  • Arthritis; 
  • Cachexia (condition that causes weight loss and muscle loss); 
  • Cancer; 
  • Chronic Pain; 
  • Glaucoma; 
  • Migraine; 
  • Continuous muscle spasms; 
  • Seizures; 
  • Severe nausea; or
  • Any other chronic or lasting symptom that either: 
    • Limits your ability to do one or more major life activity; or
    • May cause harm to your safety, physical, or mental health if not relieved.

How to Apply

1. Call us (the Office of Vital Records and Statistics) at (619) 692-5723 to schedule an appointment. OVRS is:

2. Have these ready:

  • A completed Medical Marijuana Program Application/Renewal Form.
  • A valid government-issued photo ID.
  • Proof of residency in San Diego County, such as:
    • Rental or mortgage agreement,
    • Utility bill, or
    • California DMV motor vehicle registration.
  • Written documentation or the Written Documentation of Patient's Medical Records Form from your medical provider.
    • This documentation must include an original signature.
    • Written documentation must be on your medical provider’s business letterhead.
    • The medical condition is NOT to be listed.
  • Your medical provider’s name, contact information, and California medical license number.
  • Current Benefit Identification Card (BIC) and/or Medi-Cal card if you are a Medi-Cal beneficiary.

3. Complete non-refundable application fee of $44.00 (or $22.00 for Medi-Cal beneficiaries). Pay by:

  • Cash, Debit Card, Credit Card, tap to pay; or
  • Check, or money order, payable to County of San Diego.

4. If your application is approved, you will need to pick up your MMIC from the Office of Vital Records and Statistics.

Additional Requirements

  • Persons under the age of 18 must have a parent or legal guardian with them or show proof of emancipation.
  • Primary caregivers must be with the patient when submitting the application.

How to Appeal

If your application is denied, you can file an appeal with the California Department of Public Health (CDPH).

1. Fill out the Denial Appeals Application Form.

  • Complete and submit the appeals paperwork within 30 calendar days from the date you were notified of your application being denied.

Please sign and date your appeals form. By signing your appeals form, you are giving consent for your county program to release information in your application file to CDPH.
