K-12 Youth Services Community Grant Program Application Questions

Minimum Requirements

- Possess or in the process of acquiring all licenses,
certifications or permits required for performing the proposed
program/service

Proposed Service/Program (please respond to all of the following items, 1 - 8)
1. Select one or more of the following focus areas for your proposed program/service:
- Educational Equity and Acceleration of Learning
- Behavioral Health Needs
- Housing, Food Stability, and Poverty
- Mentorship
2. Provide a description of your proposed program, including services to be provided.
Include in your description specific information regarding WHAT services or activities will be carried out through the project/program.
- State the intended goal(s) for program/project and anticipated impact(s) of your program/project on your Target Population. A goal describes the final impact or outcome that you wish to accomplish. Use words such as decrease, deliver, develop, establish, improve, increase, produce, provide, etc.
- Provide a description of the major steps that would be required to implement your program/project.
- Identify any curriculum used in your program, if applicable
3. Provide a description of the identified need and target
population(s) you intend to serve. Also describe how you will
perform outreach and identify participants in need of supportive services.
Include in your description the anticipated demographics of WHO you intend to serve, including risk factors indicating a service or program need in the area of educational equity/acceleration of learning; behavioral health; housing, food stability, and poverty; and/or mentorship. Describe your planned outreach methods and WHY you selected the Target Population, including how you will ensure you are engaging youth and families that will benefit from your program/project.
4. Provide a description of the estimated number of youth or families to be served monthly and the total to be served during the 12-month grant period.
Include in your description the anticipated number of individuals to be served and the method of counting those individuals for reporting purposes. For example, a mentor program where 10 mentees meet with their mentors four times per month may choose to count using one of these two options:
- 10 mentees served per month, or
- 40 mentor/mentee meetings per month
5. Provide a description of where and how services will be delivered.
If services will be provided at a physical location, provide a description and the address of the location where services will be provided. If services will be delivered virtually, provide a description and the program/platform that will be used.
6. Provide a description of when services will be provided.
Describe WHEN services will be provided, including the hours of service and frequency the Target Population will be engaged in your program. Also describe any anticipated follow-up and/or aftercare to assist the participant in ongoing success after program/project completion.
7. Provide a description of the anticipated benefits or outcomes to be gained including how you will measure outcomes.
Describe and include a list of anticipated benefits and/or outcomes that support your program goals. Also include a specific outcome measure for each benefit/outcome identified that will be used to measure the overall effectiveness of your program.
Guidance on describing outcomes and outcome measures: An outcome is a specific anticipated result a program/project is intended to achieve. An outcome measure is the measurable impact the services or project had on participants. Each outcome should have the ability to be measured to determine if a program or project is effective at reaching your program goals.
For example, if the goal is to increase the number of participants who obtain employment as a result of your program, a sample anticipated outcome/benefit could be listed as “80% of program participants obtain employment and remain employed for 6 months.” Based on this, the outcome measure would be tracked and reported as the “actual percentage of participants employed for 6 months out of all program participants” (calculated as the # of participants employed for 6 months divided by the total # of participants in the program). An additional example for a quality-of -life improvement activity or enrichment program could be a survey indicating the participant’s sense of improvement or level of satisfaction before, during, and after program completion.
8. Provide the amount of grant funding requested and how funding will be spent in support of your program/service?
Provide the amount of funding requested, from $50,000 to $250,000. Please ensure that the requested funding is reasonable and plausible for the proposed use of funds. Provide justification and details for how you will spend funding in support of your program/service. For programs that intend to utilize funding as part of a long-term program or project please describe a sustainability plan for funding a program beyond the twelve (12) month grant period.
