Lead agency: Sheriff’s Office
Population of focus: People with serious mental illness and mild to moderate behavioral health concerns,
SIM intercept: 2-3, 4-5
Summary of planned action:
Jail in-reach services (Project In-Reach) currently assist people with serious mental illnesses to reenter the community from custody by working with them while they are still in jail and providing case management and connections to community-based services after they are released. This action will expand these in-reach and transition services to additional populations with moderate and mild behavioral health concerns.
Actions taken:
-
From July 2024 through September 30, 2025, Correctional Counselors referred 312 individuals to Project In-Reach; nearly a third of those referrals were provided since July 2025.
-
Neighborhood House Association visited some of the detention and reentry facilities to educate the population on the services they provide.
Next steps:
-
Project In-Reach will continue to provide pre- and post-release services in detention and reentry facilities through a coordinated effort with the reentry teams embedded in the facilities.
