Good news! Form 1095-C can now be accessed in Employee Self Service.
The County will no longer mail this form.
What is Changing?
Starting in 2026, the IRS no longer requires employees to receive a physical Form 1095-C, as it is not required for filing taxes. Therefore, the County is now making the form available electronically for all employees.
What is a Form 1095-C?
Form 1095-C was designed by the IRS to capture enough information about the employer’s offer of health insurance coverage to verify that these employers are in fact meeting the ACA requirements for minimum essential coverage.
When will I receive my Form 1095-C to complete this year’s taxes?
Form 1095-C will no longer be mailed to your home unless requested. Form 1095-C will be available in PeopleSoft Employee Self Service by March 3rdof the tax filing year.

What am I supposed to do with the Form 1095-C?
Keep it with your tax records in case you need to prove that the County offered medical insurance. You do not need to file your 1095-C with your tax return(s).
My dependents were covered under my health insurance. Why aren’t they listed on Form 1095-C?
Dependent(s) would only be listed on Part III of Form 1095-C if you had an HRA (Health Reimbursement Account), and if your dependent(s) were on record with the HRA third party administrator. Otherwise, your employer isn't required to include covered dependents on your Form 1095-C.
Dependent(s) information will be included on Form 1095-B provided by the health insurer.
Do I need to wait to file my taxes until I receive Forms 1095-B and/or 1095-C?
It is not necessary to wait for Forms 1095-B or 1095-C to file your taxes. While the information on these forms may assist in preparing a return, they are not required to file your taxes.
If I request a duplicate 1095-C, how long do I have to wait for it?
Reprints are available immediately in PeopleSoft Employee Self-Service and can be accessed while logged into a County computer.

How do I obtain a copy of my Form 1095-C if I’ve moved out of the area?
Please send an email to the County Benefits Division: DHRBenefits.FGG@sdcounty.ca.gov, with your new address. A copy of your 1095-C will be mailed to you, or an electronic copy can be emailed.
I forgot to change my address, and my Form 1095-C was returned to the Benefits Division. May someone pick it up for me?
No. For security reasons, only the person whose name is on the Form 1095-C may pick it up. To pick up the Form 1095-C, present a Government issued ID, or current driver’s license (photo ID), at 5500 Overland Avenue, Suite 370 between 9:00 a.m. and 4:00 p.m. It is best to call ahead to make sure that the 1095-C form is available.
My Form 1095-C has the wrong address. Can you correct it?
Please contact your department’s payroll unit or change the address yourself through PeopleSoft Employee Self-Service. You can access it via the following link: View Form 1095-C
Why doesn’t the amount on Line 15 of my Form 1095-C match what I paid for medical coverage?
Line 15 displays the employee’s share of the monthly premium for single coverage offered by the County. This is required even if it does not match what you actually pay. For example, if you have family coverage the amount displayed on Line 15 will still be the employee share of the cost of single coverage and not the cost of family coverage.
How do I obtain a Form 1095-C for a prior year?
Active employees should be able to obtain Form 1095-C for all years through PeopleSoft Employee Self-Service, which provides printable 1095-C forms immediately.

My Form 1095-C is incorrect. How can I get it fixed?
Please send an email to the County Benefits Division: DHRBenefits.FGG@sdcounty.ca.gov. with your employee ID and explanation of correction needed.
Will I receive a Form 1095-C if I am not an active employee?
Yes. Regulations require the County provide a form to everyone who was offered medical coverage, even if they are not an active employee. The County will send it to the last known address on file in PeopleSoft.
I worked for the County of San Diego for part of the year, and I also retired in 2025. What tax form(s) should I expect to receive?
If you meet the hours threshold to be considered full time, you will receive Form 1095-C.
