Child Health and Disability Prevention (CHDP) Program

Purpose: The Child Health and Disability Prevention (CHDP) Program helps to prevent and identify health problems by providing periodic well-child exams to children who qualify and by linking them to ongoing healthcare coverage so they will grow up healthy. The CHDP Program is required by the following authorities: Health & Safety Code, Sections 104395, 105300, 105305, 120475, and 124025 through 124110; California Code of Regulations Title 17, Sections 6800 through 6874, and Title 22, Sections 51340 and 51532.

About CHDP

The CHDP Program prevents and identifies health problems through periodic, no-cost well-child exams to help children who qualify grow up healthy. CHDP Gateway links eligible children to ongoing healthcare coverage through Medi-Cal.

Services

CHDP well child exams follow the recommendation of the Bright Futures/American Academy of Pediatrics (AAP) Periodicity Schedule. For more information on the services at each well child visit, see Bright Futures/AAP Recommendations for Preventive Pediatric Health Care Schedule.

CHDP exams are comprehensive and consist of the following:  

  • Review of health history
  • Complete physical exam
  • Oral health assessment
  • Measurements
  • Sensory screenings
  • Developmental/Behavioral health screenings
  • Immunizations
  • Tests/procedures according to age
  • Health education/anticipatory guidance

Eligibility

To receive CHDP services, children and youth must live in California and meet one of the following requirements:

  • Up to 21 years of age and on Medi-Cal.
  • Up to 19 years of age and from a low-income family.
  • Between the ages of 3 and 5 years and attend a Head Start or state preschool.
  • In foster care.

Trainings Offered

The CHDP Program offers the following trainings for medical providers and school staff:

  • Provider In-Services
    • What: Training covers the CHDP Program overview, screening requirements, Gateway process, and billing tips. Information is available regarding health education topics, resources and related programs. 
    • Who: Medical providers and their staff.
    • When: The first Wednesday of every other month.
  • Training on School Entry Health Requirements
    • What: Discussion of requirements and how data should be recorded.
    • Who: Training is open to all school staff and childcare providers.
    • When: Annually and when requested.
CHDP also hosts periodic in-service trainings on a variety of topics for CHDP providers and their staff, as well as community professionals.
