Purpose: Promote school readiness and academic achievement through preventive healthcare.

Services

The Child Health and Disability Prevention (CHDP) Program Health Promotion unit works closely with schools to promote the following two required school entry health assessments:

  • School Entry Health Exam Requirement: All incoming first grade students are required to have had a well-child health exam within 18 months prior to or 90 days after entry into first grade. No cost health exams are available to children in low-income families. 
  • Oral Health Assessment: California signed into law the Oral Health Assessment requirement (AB 1433) in 2005. AB 1433 requires that children entering public school for the first time (at either kindergarten or first grade) have an oral health assessment performed by a California licensed dentist or registered dental hygienist within 12 months prior to or by May 31 of the child's first year in public school. The ultimate goal of the legislation is to connect children with a dental home.

CHDP is no longer collecting School Entry Health Exam data or Oral Health Assessment data from schools or school districts. Schools are still required to collect the certificate of health examination or waiver and retain in the student’s health record. Additionally, schools are still required to collect Oral Health Assessment Forms by May 31 and report totals to their district. School districts must forward the data to the San Diego County Office of Education (SDCOE) by July 1 of each year. 

About CHDP's School Entry Health Requirements

  • School Entry Health Exam Form: This form is to be used to document the health exam and should be returned to the child's school.
  • Kindergarten Oral Health Assessment (KOHA) Form: This form is to be used to document the Oral Health Assessment and should be returned to the child's school. 
  • Health Exam Handbook: Provides information for school staff about the CHDP Health Exam requirement and procedures. 
  • KOHA Resource Guide: Provides information for school staff about the Oral Health Assessment requirement and procedures.
  • Order School Entry Health Exam (green) and Oral Health Assessment (pink) forms  ONLINE.
  • Annual Health Exam Report for Districts and Schools no longer needs to be reported to your district, the local office of education or CHDP.
  • KOHA School Summary Tool: This tool will assist in compiling student level data.
  • KOHA District Summary Tool: This tool will assist in compiling school level data.
