What are Community Supports?
- Community Supports are free services provided by your Medi-Cal Managed Care Plan (MCPs) to support your health and well-being at home and in the community.
- People do not need to be enrolled in Enhanced Care Management to qualify for Community Supports.
- Community Supports are provided by Community Based Organizations that are contracted with the MCPs, a list of which can be found on the 2-1-1 San Diego CalAIM website.

Housing Transition Navigation Services Housing assessment, plan, and search for housing Housing Deposits Security deposits, first month utilities, set-up fees
Housing Tenancy and Sustaining Services Support and coaching to help maintain housing
Short-Term Post-Hospitalization Short-term housing to prevent going back to the hospital
Recuperative Care (Medical Respite) Short-term housing with care to heal after being in the hospital
Day Habilitation Programs Peer mentoring to improve life skills Caregiver Respite Services A short-term caregiver to provide help to a person who has a main caregiver that needs more support Nursing Facility Transition/Diversion to Assisted Living Facilities Support for activities of daily living to help people stay in their communities or prevent going into a facility
Community Transition Services/ Nursing Facility Transition to a Home Security deposit, housing navigation
Personal Care and Homemaker Services Caregiver to assist with activities of daily living
Environmental Accessibility Adaptations (Home Modifications) Ramps, grab-bars, stair lifts, roll-in shower
Medically Tailored Meals/Medically-Supportive Food Home delivered meals based on health needs
Asthma Remediation Air filters, HEPA vacuum, pest management, mold removal
Sobering Centers A safe place for people to recover who are under the influence in public for up to 24 hours Transitional Rent*
*Availability for this benefit began January 1, 2026 for Medi-Cal members with behavioral health needs.

Who is eligible for Community Supports?
To be eligible for Community Supports a person must:
1. Have active Medi-Cal and be enrolled in a Medi-Cal Managed Care Plan (Blue Sheild Promise, Community Health Group, Kaiser Permanente, or Molina Healthcare).
AND
2. Medi-Cal members must meet the eligibility criteria to receive Community Supports. Each of the 15 Community Supports have different criteria. Please talk to your health care provider or call your Medi-Cal Health Plan to see if you may be eligible.

Getting Connected to Community Supports
There are several ways to get connected with Community Supports:
- Talk the member's Health Care Provider or call the

- If you are a
Medi-Cal member, or those caring for someone on

- OR, Complete and Send a Referral Form using these steps:
- Confirm the applicant has active Medi-Cal and identify the assigned Managed Care Plan.
- Ensure the individual meets the eligibility criteria for the Community Support service that is being requested. Additional guidance can be found in the Community Supports Policy Guide Volume 1 & Community Supports Policy Guide Volume 2.
- Complete the referral form and send to the appropriate Medi-Cal Managed Care Plan. Please note, referral forms vary by Medi-Cal Managed Care Plan.
