County-Operated Intensive Case Management
Intensive Case Management (ICM) services support recovery for people who are managing ongoing mental health conditions by identifying and coordinating a range of resources to help people achieve their goals. Learn more about ICM services on the webpage.
In-person services are primarily delivered in the field at the person's preferred location but participants may be seen in the office on occasion.

*As clinically appropriate, clients of these programs will transition to Telecare AgeWise program upon turning 60 years of age.
