Assertive Community Treatment and Wraparound Programs

Behavioral Health Services offers two types of intensive outpatient care for people with serious mental health needs:

Assertive Community Treatment

Assertive Community Treatment (ACT) services support people living with complex and significant behavioral health needs. ACT may be a good fit for those who have had:

  • Past psychiatric hospitalization and emergency room visits
  • Residential treatment
  • Involvement with the criminal justice system
  • Experience with homelessness
  • Trouble engaging with traditional outpatient services

ACT is an Evidence Based Practice designed to promote recovery by helping people manage the symptoms of their behavioral health conditions. It also helps them build the skills they need to live successfully in the community, such as finding and keeping a job, maintaining stable housing, and developing strong social connections.

Services are provided by a community-based, multidisciplinary team that includes a psychiatrist or prescriber and other licensed or credentialed mental health practitioners who work alongside peer support specialists, and other practitioners. The level of services are tailored to each person and may be updated over time as needed through regular assessments and meetings with their ACT team.

To see if ACT services are right for you or a loved one:

ACT services are for adults who are:

  • Living with a serious mental illness and may also have a co-occurring substance use condition
  • Experiencing homelessness or at risk of homelessness
  • In need of intensive mental health services

In addition to ACT, there are other outpatient mental health services for adults with higher levels of need. If you’re a behavioral health provider, all referrals to ACT programs begin by submitting a Referral Form located on the Optum Single Point of Access website. Call 1-800-842-2652 or email SD_ReferralCenter@optum.com during Monday-Friday, 8 a.m. - 5 p.m., for more information.

ACT programs provide direct services 7 days a week and crisis support 24 hours a day.

Wraparound Services for Children and Youth

Similar to ACT services, "wraparound services” support children, youth, and their families who need more intensive outpatient care with services provided in the community or in their home. Unlike traditional programs that focus on problems, wraparound programs focus on each person's needs in the family and builds on strengths. A team works closely with the youth and their family to help them reach their goals and improve well-being. The goal of this approach is to avoid more intensive care or to help someone return home after receiving higher levels of care.
