Community Health Assessments

The County of San Diego Health and Human Services Agency (HHSA) strives to create a healthy, safe, and thriving community for its many residents. The regional community presentations, Community Health Assessments, and Community Health Improvement Plans on this page assess the health status of each of the six HHSA regions by determining the root causes of health that influence their residents, such as health behaviors, social factors, health services, and policy change.

Regional Community Health Assessment (CHA) Presentations

Each year, the Community Health Statistics Unit visits the five Health and Human Services Agency (HHSA) Regions’ Community Leadership Teams (CLTs) to present a wide variety of data, including sociodemographic, lifestyle, and health data. These CHA presentations inform their HHSA region's Community Health Assessment (CHA), a collaborative effort to gather information, educate, and mobilize communities, develop priorities, secure resources, and plan actions to enhance regional health.

In 2025, Behavioral Health Services began meeting with the five Health and Human Services Agency (HHSA) Region’s Community Leadership Teams (CLTs) as part of the Behavioral Health Services Act (BHSA)’s Integrated Plan to incorporate stakeholder engagement into the local community planning process (CPP). In addition to supporting the Integrated Plan, BHS meets with the regional CLTs to work with our local health jurisdiction (PHS) to support the Community Health Improvement Plan (CHIP) and Community Health Assessments (CHA) by providing analysis, interpretation and presentation of findings on local behavioral health data to the regions.

Community Health Assessment and Improvement Plans

A Community Health Assessment (CHA) is a collaborative process of gathering information to educate and mobilize communities, set priorities, secure resources, and plan actions to improve regional health. It sets the foundation for developing a Community Health Improvement Plan (CHIP), which consists of five Community Enrichment Plans (CEP).

The CHIP outlines an action plan to address key areas based on the health, safety, and well-being needs identified by the CHA. It helps health officials and community members work together to set priorities, allocate resources, and implement projects and programs to enhance community health and wellness over the long term.
